Individual & family plans
There’s no such thing as a “typical” day with diabetes. Some days feel manageable. Others feel like a nonstop mental checklist: Did I pack my meter? What was my last A1c? Is this stress throwing everything off? It’s a lot, and it can feel like you have to fix everything all at once.
But you don’t. You don’t have to overhaul your life. You don’t have to do it perfectly. And most importantly, you don’t have to do it alone.
Moda Health’s specially trained coaches support members living with diabetes every day. They meet you where you are, with compassion and understanding. Whether you’re newly diagnosed or have been managing diabetes for years, our coaches are trained in behavior change and emotional support. And as you learn new ways to manage your diabetes and improve your health, you may find that you’re saving money on prescriptions, supplies and office visits.
You don’t have to be ready to run a marathon; if walking around the block once a week feels like the right place to start, that’s where we’ll begin. Coaches work with you by phone to set small, realistic goals that fit your life.
“Coach Alex told me I was strong enough to do this, and that really stuck with me,” said one Moda Health member. “It’s hard to believe my first goal, eating one vegetable a week, turned into eating mostly veggies and lean chicken. I’ve lost 80 pounds. I go to the gym. I can play on the floor with my grandkids.” Our coaches meet you where you are: no judgment, no pressure. Just one realistic step at a time, at your pace.
The first step is identifying what matters most to you. That might be having the energy to work in the garden, learning to cook differently or finding new, fun ways to move your body. Your priorities guide our coaching, and we’ll walk alongside you, helping you take the small steps that can lead to big changes.

If you’ve been diagnosed with diabetes, you may be eligible for Livongo by Teladoc, a digital program that helps you manage your diabetes with less stress and no-cost supplies.
With Teladoc, you get a smart meter, unlimited test strips and lancets and real-time support whenever you need it, all at no cost to you.
You’ll also receive a connected scale and blood pressure monitor, so you can track your health and easily share information with your provider at your next appointment. Whether you love data or just want peace of mind, Teladoc makes it easier to monitor your blood sugar and know whether your current plan is working.
“The biggest surprise for me was how the graphs helped me understand my blood sugar spikes and dips,” said one Teladoc user. “With free supplies, I never hesitate to check my blood sugars. Being able to filter by meal – and see trends, notes about my stress, sleep, or whether I’ve taken a walk – helps me really understand what’s happening in my body. Before, I used my numbers to decide how much insulin I needed. Now, they help me understand my overall health and make different choices throughout the day.”
