Continue your coverage with a Moda Health Conversion Plan

Did you know that even if you lose your group health coverage, you may still be able to continue having health coverage through Moda Health? If you are an employee or member whose coverage terminated under a Moda Health group medical benefit plan, you may be eligible to enroll in a Moda Health Conversion Plan.

All of your dependents may also get coverage under the Moda Health Conversion Plan, provided that your dependents were covered under your prior plan. Dependents include your spouse/registered domestic partner and dependent children.

You are eligible to enroll for conversion coverage if you meet the following requirements:

Dependent enrollment and eligibility
To enroll your eligible dependents for conversion, they must be included on the application and they must not be covered by Medicare. If accepted by Moda Health, your dependents will be included in your policy.

Your legal spouse or domestic partner is eligible for coverage. Your children are eligible until their 26th birthday.

Automatic payments
It’s easy to set up automatic payments for your coverage, so you don’t have to worry about remembering your premiums every month. Just submit an electronic funds transfer form.
