Access to food, housing, transportation, and social support can make a big difference in daily life. That's why Moda Health created the Community Resource Bridge program—to help connect members with local resources that can provide support when they need it most.

Getting support for your daily needs
Research suggests one in five people will experience at least one basic unmet need, and it can affect anyone, regardless of age, gender, race or ethnicity.
When you enroll in the Community Resource Bridge program — included with your health plan at no additional cost — you’ll work with a Community Resource Advocate. They listen, ask questions to better understand your needs and connect you to helpful resources and services in your community.

Food resources
If you’re recovering from surgery, facing challenges accessing healthy food, or finding it difficult to afford the groceries you need, a Community Resource Advocate can help you explore available options. Depending on your situation, they may connect you with food delivery services, food co-ops, community cupboards or specialized nutrition support for chronic health conditions.

Housing resources
There are many reasons our members might need support feeling safe and comfortable at home. Whether it’s an unusually high energy bill, a rent increase, unhealthy conditions like mold or poor water quality, or experiencing housing instability, Advocates can help you explore your options and connect you with available resources. And if you’re experiencing domestic violence, an Advocate can help connect you with services and housing resources that support your safety and well-being.

Transportation resources
Getting where you need to go can affect many aspects of your daily life. In addition to helping you find transportation to provider appointments or the pharmacy, Community Resource Advocates can also connect you with resources to help you get to work, school and other essential destinations. This may include passes for public transportation, shuttle services, ride programs and other transportation assistance.

Social resources
Staying connected to others can have a positive impact on your overall well-being. Community Resource Advocates can help you find local groups, clubs, activities and support networks that match your interests and needs. They can also help you identify mental health resources and connect with peer support groups, community events, outdoor activities such as hiking clubs, and indoor activities like tabletop gaming, bowling and more
Community Resource Advocates
When you enroll in the Community Resource Bridge program, you’ll receive friendly, judgement-free support from a real person who gets to know you — offering personalized guidance and helping connect you with resources that fit your needs.
Our Community Resource Advocates are trained in trauma-informed care, cultural awareness, member education and more. They’re here to support members of all ages, backgrounds and life experiences, including members who speak languages other than English and members of the LGBTQIA+ community.
Asking for help isn't always easy. Community Resource Advocates have established relationships with local programs and services and can help simplify the process of finding support. Whether you're not sure where to start or need help navigating available resources, your Advocate can help connect you with the right support.
