What if getting the right help could turn a strained relationship into a happy one? Not long ago, a Moda Health member who had been diagnosed with depression signed up for the Disease Management coaching program. His condition was starting to affect his marriage. To make things worse, his short-term memory loss made it hard for him to find a therapist and remember to attend appointments.

During their weekly meetings, the coach helped by reminding him to schedule — and attend — his appointments. He started couples therapy with his wife and began going on date nights. After a few months, the coach received an update: a photo of the couple eating ice cream together and smiling.

It’s not what most people would expect from health coaching, especially from an insurance company. But Moda Health designed its disease management health coaching program around the idea that everyone’s coaching needs are unique. It bridges the gap between getting a diagnosis and finding a care plan that works by focusing on one question:

“How can I make my care plan work in real life, not just on paper?”

Personalized and pressure-free

Putting pressure on a member to change their habits doesn’t work. So many of us are told to exercise, make changes to diet or manage stress and are left asking, “How am I supposed to do that?”

Moda Health’s Disease Management program takes a personalized approach. Eligible members work with professional, board-certified coaches who understand that two people with the same diagnoses will likely have very different health goals and journeys getting there. Their focus? Meeting you where you’re at. That means exploring what the next step might be and never pressuring you to do anything.

It starts with an interview. Your coach will ask you lots of questions to help understand your goals, obstacles and what motivates you. In fact, Moda Health coaches are experts in behavior change. They won’t tell you what to do. They’ll coach you toward finding the answer for yourself, and then help you figure out how to make it happen.

While your provider might not have enough time to discuss your mindset, coaches can gently guide you to explore what motivates you. Maybe it’s playing with your grandkids, getting out of bed without pain or having more energy throughout the day.

Imagine going from storing your bike in the garage where it’s gathering dust to taking a 50-mile ride. For one member who was dealing with chronic pain, Moda Health coaching got her there in a few months. She had been passionate about offroad biking but had to give it up because of pain. After gaining the motivation to work through her physical therapy exercises, she was able to get back on her bike and rediscover one of her favorite hobbies.

Exploring the many paths to health

Health coaching works gradually over time. You can expect to spend about five to ten months in the program, meeting with your coach once every week or two by phone or video.

The program works in three parts as you:

  1. Learn about your condition and its impact on your health.
  2. Understand what it takes to manage your condition, including lifestyle changes.
  3. Explore the many different paths to making these lifestyle changes.

Coaching works by focusing on how to get what you want. Coaches emphasize that taking care of yourself is not about going to the gym three times a week or eating salads. In fact, they’ll encourage you to challenge the social norms around healthy habits. Real change could mean taking a fun cooking class with a loved one, walking your dog around the neighborhood and meeting new people, or putting on music and dancing while doing the dishes.

Once you start making healthy changes, you might find that other parts of your life are easier. You may have less pain or inflammation, or more energy or better sleep. That leads to the motivation to keep going and do even more.

Health coaching doesn’t replace medical care, but it does give you time and space to reflect on your best healthy life. Along the way, you’ll get support from compassionate, knowledgeable coaches who want to see you succeed.
