Patient care and diversity
Moda is committed to building a network of providers who value diversity and can deliver care that meets the unique and specific needs of our members. Today, many patients are selecting providers that align to their DEI needs. Our directory tool displays diversity data to help our members make informed decisions when selecting a provider. To help us include your diversity data, please fill out our diversity forms.
- Clinic diversity data: This form captures DEI information for your facility/clinic.
- Provider diversity data: This form captures DEI information for each contracted provider.
In-network referrals
When your patient needs specialty care or another provider, help them make the most of their benefits by referring them to an in-network provider. They can use Find Care, our online provider directory, to search for doctors, specialists and facilities covered by their plan.
