As you manage all the responsibilities in your life, you might find yourself in need of someone to help you find the right mental health care and support.
Our Behavioral Health 360 program gives you different ways to access mental health support. Whether you prefer the personal guidance of a Behavioral Health Champion or the privacy of taking a self-guided assessment, your member dashboard makes it easy to find the mental health support that's right for you.
To get started, go to the Behavioral Health 360 tile on your Member Dashboard home page or download the free Moda 360 app from your app store or Google Play and connect to care no matter where you are. Don't have an account? Creating one is easy. Go to the Member Dashboard login page and select "Create an account" to enter your information. Be sure to have your member ID card handy.

Moda Behavioral Health Champions:
Our dedicated Behavioral Health 360 Champions bring all the support and tools for mental wellness directly to you. Use Behavioral Health 360 Champions to help you find the right mental health care, services and support.
Behavioral Health 360 Champions can help:

- Verify provider availability
- Get the care and support you need; quickly and easily
- With follow-up connections to make sure you have what you need and are getting the care and support you're looking for

Behavioral Health Self-Guided Assessment
The Behavioral Health Self-Guided Assessment walks you through a series of questions to help you identify the best fit for behavioral health resources. The tool offers personalized recommendations based off your responses, crisis resources, and an invitation to contact a Behavioral Health Champion if you would like additional assistance.
- Start by logging into your Moda Health Member Dashboard online or through our app where you can find the "Behavioral Health 360 Tile."
- Once you click on the tile, you will be directed to the "Behavioral Health 360 Programs" tab where you will find a link for the "Behavioral Health Self-Guided Assessment."
- Once you have located the "Behavioral Health Self-Guided Assessment" click the link to begin and answer each question to the best of your ability. Our goal is to help you find the best recommendation for your specific concerns. If you believe you need additional support, we encourage you to contact a healthcare professional or reach out to a Behavioral Health Champion for assistance.
- Once you have completed the survey, we will recommend programs and resources that could be a great fit for you based off of your responses. You can reach a Behavioral Health Champion at 833-212-5027 or by e-mail at bhchampions@modahealth.com

Spring Health
Spring Health provides access to mental health telehealth services by using a phone, tablet or computer. Get matched with experienced providers in an average of 2 days. A clinically licensed care navigator will help you create a personalized care plan in areas you need help with, including care navigation, therapy, coaching, self-guided exercises and more. Gemiini
Gemiini provides resources and support for children with Autism Spectrum Disorder and other developmental disorders. Gemiini's discrete video modeling program is an evidence-based tool that helps children develop language and behavioral skills. Gemiini coaches are also available to help families make the best use of the tools, and help parents find additional resources to maximize their child's education and development.

Hazelden Betty Ford
Substance use treatment and resources
Hazelden Betty Ford Foundation offers an array of resources to help you and your family recover from substance use disorders. Hazelden's "Connection" program offers case management for 11 months following step down from residential treatment to help establish ongoing recovery. Hazelden also offers education for family members, including how to identify substance use concerns in teens and what to do when a family member does not want to seek treatment.

Meru Health
Get therapy on your smartphone through Meru Health. This completely confidential online program is a 12-week treatment program to help reduce anxiety, stress, depression, and burnout. The program offers:
- Confidential access to a personal, remote therapist
- Anonymous peer support
- Mindfulness and behavioral techniques for sleep, nutrition and more
- Wearable biofeedback training to increase focus and manage stress
