Your PEBB plan includes Providence Health System

Moda’s Connexus network gives you access to Providence facilities and providers across Oregon, making it easy to continue receiving care from the Providence doctors, clinics and hospitals you know and trust. Connexus is one of Oregon’s largest provider networks, connecting you to more than 30,000 doctors and 87 hospitals across Oregon, Idaho, Alaska and Southwest Washington. Always confirm that a provider or facility is in-network before receiving care.

Two medical plan options to fit your needs

• PPO plan: Offers broad access to in-network providers and flexibility when choosing care. Also available to members who live outside Oregon.
• CCM (Coordinated Care Model) plan: Built around a PCP 360 primary care provider who helps coordinate your care and connect you with the services you need. No referrals are required to see specialists.

The PCP 360 model is designed to simplify your healthcare experience by putting your primary care provider at the center of your care team. Your PCP helps coordinate care across providers and services, supporting a more connected and personalized healthcare journey.

Providence is a PCP 360 provider

If you currently see a Providence primary care provider, you can continue that relationship through the PCP 360 program beginning January 1, 2027. Providence providers will be listed as PCP 360 providers starting October 1, 2026. You can also use Moda’s Find Care directory or the Moda 360 Member Dashboard to:

• Search for participating providers.
• Confirm network participation.
• Verify whether a provider is accepting new patients.
