A Medicare Companion plan made for you

Our Medicare Companion plan was created for PERACare retirees. It works alongside Original Medicare and helps cover many of the out-of-pocket costs that Original Medicare doesn’t.

Our plan offers coverage similar to a Medicare Supplement (Medigap) Plan G, with the freedom to choose any Medicare provider anywhere in the U.S. and U.S. territories.

For 2027, this plan includes helpful programs and services at no extra costs, including:

• 24/7 Nurse Advice Line: Talk with a registered nurse anytime, day or night
• One Pass® fitness program: Get access to fitness centers, classes and wellness resources
• Travel assistance: Get help when you’re more than 100 miles from home through Assist America

The Medicare Companion plan is administered by Moda Health (Moda) and includes Medicare Part D prescription drug coverage administered by Humana. The plan is available to retirees who reside in the United States or its territories.

If you are enrolled in the Moda Medicare Companion plan, you are also enrolled in the Humana stand-alone PDP plan. You can fill prescriptions at participating retail pharmacies nationwide or through CenterWell Pharmacy, Humana’s mail-order pharmacy. Specialty medications can be filled through CenterWell Specialty Pharmacy, Humana’s specialty pharmacy.
