Moda Health’s maternity program, Growing Together, strengthens your care for pregnant patients by providing additional support between check-ins. This program complements your clinical care while helping manage common questions and concerns that arise during pregnancy and early parenthood.

Available at no cost to Moda Health members, our maternity program helps reduce the time you spend addressing routine inquiries while ensuring your patients stay better informed and engaged with their care plan.

When members opt in to the program, we support your care through:

  • Enhanced patient support between visits: Health coaches are available to reinforce your care instructions and help patients stay on track with appointments and preventive care. They can spend time addressing common concerns, allowing you to focus on medical priorities during appointments.
  • Proactive education and monitoring: Patients receive cadenced information aligned with their stage of pregnancy. Coaches check in regularly to identify potential concerns early, helping you address issues before they become serious.
  • Clinical care reinforcement: Out team supports your treatment plans by providing supplemental guidance on nutrition, birth preparation and lactation.
  • Streamlines community resource navigation: We help connect patients with local support services, reducing the administrative burden on your staff while ensuring patients can access needed resources.
