Plan ratings

The Medicare program rates how well plans perform in different categories (for example, detecting and preventing illness, ratings from patients and customer service). You can visit www.medicare.gov and compare drug and health plans or compare Medigap policies to get Medicare plan ratings in your area. You can also call Moda Health directly at 888-786-7509 (TTY users call 711) for a copy of our plan ratings. PERS ODS Advantage Rx plan is a 4.5-Star rated plan. Plan performance summary star ratings are assessed each year and may change from one year to the next.

Potential for contract termination

Moda Health Plan, Inc.'s contract with the Centers for Medicare and Medicaid Services (CMS) may possibly not be renewed each calendar year by CMS or by the plan. The benefits, formulary, pharmacy network, premium, service area, copayments and coinsurance may change on January 1, 2013. You will be notified in advance of any changes that may occur. Coverage beyond the end of the current contract year is not guaranteed.
