Gender-affirming care
Moda Health is committed to delivering compassionate and affirming healthcare for individuals who identify as transgender or non-conforming. Our dedication extends to connecting members with safe, compassionate providers who possess specialized knowledge in gender-affirming care. We recognize the importance of comprehensive and integrated gender-affirming care across the healthcare spectrum, ensuring a supportive and understanding healthcare journey for our members.
Your gender-affirming care benefits, based on the World Professional Association for Transgender Health (WPATH) guidelines, ensure you have access to medically necessary services and treatments that include, but are not limited to:
- Office visits and consultations
- Behavioral health therapy
- Hormone therapy
- Breast surgery
- Genital surgery
- Facial surgery
- Medically necessary hair removal
- Tracheal shave
- Voice modification therapy
- Services for the removal and preservation of oocytes and sperm prior to the treatment of gonadotoxic therapy

The Member Dashboard features a Behavioral Health Self-Guided Assessment, Behavioral Health Resources, and a contact form to reach the Behavioral Health Champions. These tools and more are now available on our app by downloading the Moda 360 mobile app from your app store or visiting our website and logging into your Member Dashboard.
Self-Guided Assessment
Find the right care and support for you is important. The Self-Guided Assessment tool walks you through a series of questions to help you identify the best fit for behavioral health resources. The tool offers personalized recommendations based on your responses, crisis resources, and invites you to contact a Behavioral Health Champion if you would like more help.
To get started:
- Log in to your Member Dashboard.
- Click on the "Behavioral Health 360 programs" tab.
- Find and select the "Self-Guided Assessment" link.
Spring Health
Provides access to mental health telehealth services by using a phone, tablet, or computer.
Meru Health
Online 12-week program to help reduce anxiety, stress, depression and burnout.
Hazelden/Betty Ford
LGBTQIA-specific treatment for addiction and substance use issues, including programs for teens. Inpatient and outpatient treatment programs are available.
Cyti Psychological
Online therapy with LGBTQIA-friendly providers for individuals, couples, and families. A gender-affirming care team is staffed with professionals trained in gender identity and sexual health.
Value-added services are a complement to the plan but are not insurance. Benefits and services vary by plan.
