Moda 360 Pre-D prevention program

Nearly 1 in 3 Americans have prediabetes, and of those 80% don't even know they have it. We created the Pre-D prevention program — which follows a CDC-backed curriculum and format — to help you identify your risk of type 2 diabetes. First, you take the screening survey. Then, we provide professional support and resources at no cost to help you feel your healthy best. Making changes today may help you avoid type 2 diabetes, which can impact your daily and lead to long-term health impacts.

Support and community

When you join the Pre-D prevention program, you receive tools, resources, coaching and community to help you reach your health goals.

  • Waved co-pays and deductibles for in-network nutritionist or dietitian visits
  • 22 virtual, real-time group coaching sessions with a certified health coach
  • A free Fitbit and smart scale to track your movement and weight
  • The skills you need to lose weight, be more physically active, and manage stress
  • Support from other participants with the same goals as you

Your coach will see you through your continuum of care by providing ongoing guidance and support. They can connect you to programs like WeightWatchers for weight loss and Meru Heath for mental health.

Program materials and translation services are available in 14 other languages, including Spanish. Reach out to our team for support in getting you the materials you need.

How it works

  1. Prediabetes screening
    If you are 18 years or older and an OEBB member, you can complete the online screening survey for prediabetes, designed by the CDC.

  2. Introductory call
    If you qualify for the Pre-D program, you'll receive an introductory call from a Moda Health Coach. Your coach will explain the program, its benefits, and the requirements to participate, which include completing the onboarding process.

  3. Onboarding call
    Your health coach will answer any questions you have about the program, go over your health history and work with you to set goals for the next 12 months.

  4. Group coaching sessions
    12-month coaching program led by a trained Lifestyle Coach with 22 virtual, real-time group coaching sessions. You and other participants will learn a CDC-approved curriculum with techniques to help make lasting lifestyle changes.

  5. Quarterly check-ins
    Your Moda Health Coach will go over your progress and engagement in the program. They will also provide recommendations for nutritionist visits and/or A1C lab tests.

  6. Disenrollment
    You can disenroll in one of two ways. First, you and your Health Coach can determine if you have graduated and don't need any follow-up visits. Or you may choose to no longer participate. At this point, your nutritionist visit copay will be set back to standard cost sharing.
