Vision benefits and plan materials

Taking care of your eyes is about more than getting a prescription for eyeglasses or contacts. Regular vision care helps protect your eye health and can catch early signs of any problems. Your OEBB vision plan makes it easy to see your eye doctor every year.

Basic information about your vision benefits

There are a few things to know about how your vision coverage works:

  • Your plan covers:
    • One routine eye exam every plan year
    • One pair of lenses every plan year for all ages
    • One pair of frames every plan year for children under 17 and one pair of frames every two plan years for adults age 17 and older
  • All tints and prescription lenses are covered.
  • Polarization and non-prescription sunglass lenses are not covered.
  • Your plan maximum is the most Moda Health will pay and is based on the plan you choose. You can find your plan maximum in your member handbook, which you can find below.

Using providers in your network

You can find participating providers using the Find Care tool. These providers won’t bill you for any difference between the contracted rate and their usual rate.

The “plan allowable” is either the contracted rate that participating providers agree to accept as payment in full or the charges billed by nonparticipating providers.

Double coverage under two OEBB plans

If you are covered by two OEBB plans, you typically have the same frequency limits as those who are covered under one OEBB vision plan. However, you may have a higher benefit maximum for covered vision services.

  • Vision claims are processed under your primary coverage. If there’s an additional benefit to pay, we’ll also process claims under your secondary coverage automatically.
  • If you don’t want to coordinate your vision benefits on dual OEBB plans, call Moda Health in advance. We can process all vision claims (exam and hardware) on one plan one year and all vision claims (exam and hardware) on the other plan the next year.
  • We cannot process hardware on one plan and the exam on the other.
  • If you call in the middle of your benefit period and we already have claims on file for that plan year, your request will not start until the next plan year.
