1This program is not insurance. Moda Health Plan, Inc. (Moda) is not affiliated with Garner. Moda has contracted Garner for this service. Moda does not aid in data collection or provider scoring and is not responsible for any reimbursement from utilizing Garner Top Providers and does not administer HRAs.
2If members are on High-Deductible Health Plan (Medical Plans 6 and 7), they must first meet the IRS minimum annual deductible of $1,700 for a self-only plan and $3,400 for a family plan before they can use the Garner incentive.
3If you have an HSA, FSA, or HRA, you cannot be reimbursed by both Garner and your HSA, FSA, HRA for the same visit. That’s considered “double-dipping,” which violates IRS rules and could result in tax penalties.
