Registered Nurse Advice Line

The Registered Nurse Advice Line allows you to get answers and information about your health over the phone, day or night. Nurses can help you with basic health situations, such as:

Care coordination

Care coordination registered nurses work directly with facilities and providers to facilitate preauthorization of scheduled procedures, inpatient stays, and follow–up care as needed for members during urgent or emergent admissions. During the time a member is receiving care or is hospitalized, care coordination nurses may refer them to a case management nurse if they require additional assistance and coordination for complex or catastrophic conditions.

The hospital calls Moda Health to notify us of all urgent/emergent hospital admissions within 48 hours, or as soon as possible.

Case management

Case management is a voluntary service for members experiencing complex conditions or catastrophic events and need assistance from a case management registered nurse or behavioral health specialist. Case managers can help by working with members and their families as patient advocates to:

If you want to make a referral to case management, please call Moda Health Healthcare Services case management at 503–948–5561 or toll–free at 800–592–8283. You can also email, fax a referral to 503–243–5105855–232–6904 or complete and submit our online referral form. A member can self-refer to case management or be referred by a family member, caregiver, provider or facility staff. To make a referral, you’ll be asked to provide the following information:

Once a referral is received, a case manager will evaluate the member’s situation and contact the member within five (5) business days.

Moda Health offers in–depth support programs for those dealing with chronic health conditions. You have access to tools and resources that help you maintain a healthy lifestyle. Individual health coaches provide you with one–on–one support. These specialized programs include:
